Hello,

YOU ARE HEREBY GIVEN NOTICE that under the term of that lease dated <date> between <landlord> as Lessor, and yourself as Lessee, for the premises known as <Full address of leased property>, you are in default of said Lease for the following <reason(s)>:

Failure to pay rent when and as due as stipulated in said Lease as follows:

- <list of overdue payments>

Total rent now payable amounts to $<amount due>.

In the event of your failure to either cure said defaults or vacate the premises covered under said Lease within <grace period> from and after your receipt of this Notice, you shall be in unlawful detainer of said premises. In the event of your failure to either cure said defaults or vacate the premises by said date, you shall be subject to Lessor's rights to commence an action for unlawful detainer as provided by applicable law. In any such action, Lessor shall seek both possession of the premises and monetary damages, including prejudgment interest on each of the amounts set forth above, double the amount of rent past due, attorneys fees and costs.

Should it be necessary to evict you and your property remains on the premises, please be advised that the law allows that Lessor shall be entitled to reasonable storage costs of your property until such property may be removed.

In order to cure these defaults you must, within the time set forth in this Notice, pay all sums set forth above to Lessor by tender to Lessor at <full address of lessor>.
