LEASE RENEWAL AGREEMENT
<Name of lessor> ("Lessor") and <Name of Lessee> ("Lessee") entered into a prior lease agreement, dated <date of lease> ("Original Agreement").
Lessor and Lessee desire at this time to further extend and amend said Original Agreement as hereinafter set forth.
It is therefore mutually agreed that the "Original Agreement" shall be further extended and amended as follows:
The term of the lease shall be extended to <new end date>.

All of the terms and provisions of said "Original Agreement" except as herein modified, are to remain in full force and effect and are herein incorporated by reference as part of this Renewal
Agreement.
